چکیده
Abstract Introduction The association between atrial fibrillation (AF) and chronic kidney disease (CKD) is directly proportional carries a higher risk of death, bleeding, thromboembolism (TE). There uncertainty on the clinical benefit direct oral anticoagulants (DOAC) in patients with advanced stages CKD. Purpose To perform systematic review meta-analysis available literature efficacy safety DOAC compared to warfarin AF stage IV V CKD or without hemodialysis. Methods We reviewed PubMed, SCOPUS, EMBASE, LILACS, Google Scholar, Open Gray, Clinical Trials until April 30th, 2021. A standardized search was performed independently by 2 authors. primary endpoint reduction TE (stroke systemic embolism). outcome major additional bleeding outcomes were analyzed as well. included randomized controlled trials observational intervention studies. Results data from 80,771 participating 14 studies (2 sub-analyses 12 studies). bias low, moderate, high 2, 6, 6 studies, respectively. did not find any difference use versus reduce TE; there trend but non statistically significant though (RR: 0.88 [95% CI, 0.72–1.06], I2=18.7%). Atrial Fibrillation and Oral Anticoagulation in Chronic Kidney Disease.
Due to several unfavorable epidemiological changes, chronic kidney disease (CKD) and treatment of its associated cardiovascular morbidity have become a worldwide problem. Thus, atrial fibrillation (AF) is the most common arrhythmia and frequently associated with renal impairment: prevalence for AF is up to 27% in long-term hemodialysis patients and in general more than 25% in all CKD patients 7...

Background: Pentoxifylline, a valuable medication with promising clinical characteristics and considerable profile of safety is used in many conditions namely chronic kidney diseases (CKD). However, the decision to prescribe pentoxifylline for anemia in CKD should be based on evidence accrued from randomized controlled trials (RCTs). Yet, substantial heterogeneity exists in studies performed to...

متن کاملPatients’ and physicians’ perceptions and attitudes about oral anticoagulation and atrial fibrillation: a qualitative systematic review
BACKGROUND Oral anticoagulant therapy reduces the risk of stroke in patients with atrial fibrillation, but many patients are still not prescribed this therapy. The causes of underuse of vitamin K antagonists oral anticoagulants are not clear but could be related, in part, to patients' and physicians' perceptions and attitudes towards the benefits and downsides of this treatment. The purpose of ...
